Rassie van der Dussen and Dale Steyn will be rested and Andile Phehlukwayo and Junior Dala will come into the squad.
Dwaine Pretorius will be pushed up the order to No 3 so that he can have a chance to prove himself with the bat.
Aiden Markram will have a final chance to prove that he deserves a spot in the World Cup XI. He failed to take his opportunity in the second T20I and lost his wicket cheaply when he found himself in the dugout early in the innings, only to watch Hendricks play an admirable knock that set up the series win.
After the toss Proteas skipper JP Duminy said he would have elected to bowl first, while Sri Lanka skipper Lasith Malinga announced that his side had made one change to their side. The visitors have opted for the extra seamer by bringing Suranga Lakmal back into the side, dropping star batsman Kusal Menis to make way for Lakmal.
